This is the commonest cause of pylephlebitis of the liver. This is a case of perforated
appendix with generalized peritonitis. The arrow at the bottom points to the perforation
in the appendix and you can see a yellowish-green, fibrinous
covering over the small intestines at the bottom of the specimen.
