Ventricular septal defect
This is the most common congenital anomaly of the cardiovascular system, the ventricular septal defect. These are usually in the high membranous portion the septum as is the case here. Small defects such as this one may cause a loud systolic murmur, but can be asymptomatic. This case was a 51 year old man who died of renal disease and no cardiac symptoms.
